I am writing to refer a 6-year-old female patient for further evaluation and management of a concerning presentation involving severe thrombocytopenia. The patient has recently exhibited symptoms consistent with an upper respiratory tract infection accompanied by a persistent fever over the past week.

Clinical Background and Presentation:

  • Medical History: Previously healthy with no significant past medical issues.
  • Current Symptoms:
    • Fever and Respiratory Symptoms: Reported for approximately one week, suggesting an acute upper respiratory tract infection.
    • Rash: A diffuse petechial rash is present over the trunk and upper limbs.
  • Laboratory Findings:
    • Platelet Count: Notably low at 8,000/µL, indicating severe thrombocytopenia.
    • Other Complete Blood Count (CBC) Parameters: Remain within normal limits.

The presence of severe thrombocytopenia in the context of an acute infectious presentation is particularly concerning and requires prompt and comprehensive evaluation to determine the underlying cause and appropriate management strategy.

Examination Notes:

  • There were no additional signs of bleeding, hepatosplenomegaly, lymphadenopathy, or other systemic symptoms during the initial assessment.

Given the severity of the thrombocytopenia and the potential for significant complications, I recommend a detailed evaluation to explore possible etiologies, including but not limited to, immune thrombocytopenia, other hematological conditions, or infectious causes that could explain her current hematologic findings and associated symptoms.

The family is understandably concerned and has been informed about the need for specialized care to address her current health issues. They are prepared for further diagnostic procedures and treatment interventions that may be necessary as part of her ongoing care.

Thank you for referring the 6-year-old female patient for evaluation of her severe thrombocytopenia and associated symptoms. We have completed an initial assessment and have implemented a management plan based on her clinical presentation and laboratory findings.

Clinical and Family History Review:

  • Symptoms: The patient presented with a history of an upper respiratory tract infection (URTI) and a noted episode of epistaxis prior to referral.
  • Family and Social Histories: No notable medical issues or relevant conditions were reported that could be linked to her current hematological presentation.

Physical Examination Findings:

  • The patient is active, well, and hemodynamically stable with no dysmorphic features noted.
  • Rash: Diffuse petechial rash over the trunk and upper limbs, consistent with observed symptoms of thrombocytopenia.
  • Respiratory and Abdominal Exam: The chest is clear, and the abdomen is soft and lax without any signs of organomegaly.

Laboratory Results and Diagnostic Testing:

  • CBC Findings: Isolated thrombocytopenia with other parameters within normal limits.
  • Based on these findings, we have initiated the following diagnostic tests and treatment:
    • Peripheral Blood Smear and Coagulation Profile: To further investigate the cause of the thrombocytopenia and rule out any malignant cells.
    • Virology Screening: Including tests for HIV, Hepatitis, Parvovirus B19, Varicella, CMV, and EBV, to rule out infectious causes.
    • Intravenous Immunoglobulin (IVIG) Therapy: Administered at 1 gm/kg/day for 2 days as an initial therapeutic approach.

The patient’s clinical presentation and initial investigations suggest a diagnosis of newly diagnosed Immune Thrombocytopenia (ITP).

  • We recommend a repeat CBC after 48 hours to monitor the stability of her platelet count and assess the need for further intervention.

Recommendations:

We will continue to monitor her condition closely, with particular attention to any symptoms that may suggest a relapse or require additional treatment. Continued follow-up will help ensure that the patient maintains stable hematological parameters and to manage any potential complications from ITP or underlying conditions that may emerge.

Discussion

Immune Thrombocytopenia (ITP) is a hematologic disorder characterized by a low platelet count and increased risk of bleeding. The primary goal in managing ITP is to ensure a platelet count sufficient to prevent significant bleeding, rather than achieving a normal platelet count. This involves a tailored approach based on the patient’s bleeding symptoms, life quality considerations, and the potential side effects of treatments.

Diagnostic Evaluation for ITP

A thorough evaluation is crucial for accurate diagnosis and includes:

  • History: Key indicators include isolated bleeding symptoms indicative of thrombocytopenia without systemic symptoms like significant weight loss, bone pain, or night sweats.
  • Physical Examination: Absence of hepatosplenomegaly, lymphadenopathy, or signs of congenital conditions alongside bleeding symptoms.
  • Complete Blood Count (CBC): Typically shows isolated thrombocytopenia with a platelet count below 100,000.
  • Peripheral Blood Smear: Platelets appear normal to large in size; red and white blood cell morphology is normal.

Bone marrow examination is generally not necessary in straightforward ITP cases. Bone marrow examination is felt to be unnecessary in children with typical ITP prior to initiation of treatment with corticosteroids, prior to splenectomy, or in patients who fail intravenous immunoglobulin therapy. The presence of abnormalities in the history, physical examination, or the complete blood count and peripheral blood smear should be further investigated, e.g. with a bone marrow examination or other appropriate investigations, before the diagnosis of ITP is made.

Treatment Strategies

Observation: Many patients, especially those with no or mild bleeding (skin manifestations like petechiae and bruising), can be managed by observation alone, irrespective of the platelet count.

First-line Treatments:

  • Corticosteroids: A short course can effectively increase platelet counts. Extended treatment does not show additional benefits and increases the risk of side effects.
  • Intravenous Immunoglobulin (IVIG): Administered at 0.8-1.0 g/kg, IVIG is preferred when a rapid response is needed, as it can increase platelet counts faster than corticosteroids. The mechanism involves the blockade of Fc receptors in the spleen, reducing platelet destruction.
  • Anti-D Immunoglobulin: Used in Rh-positive, non-splenectomized patients, Anti-D facilitates the destruction of Rh-positive red cells, distracting the immune system from attacking platelets. However, due to risks of severe hemolysis, its use is cautioned and monitored closely.

Second-line Treatments:

  • Splenectomy: Delayed for at least 12 months unless severe symptoms persist. Splenectomy removes the primary site of platelet destruction and antibody production but comes with risks of infection and surgical complications.
  • Rituximab: Targets CD20 on B-cells, reducing the production of autoantibodies against platelets. It’s considered when corticosteroids and IVIG are ineffective.
  • Thrombopoietin Receptor Agonists (e.g., Eltrombopag, Romiplostim): These agents stimulate the thrombopoietin receptor on megakaryocytes to increase platelet production. They are especially beneficial for chronic ITP management but require careful dosing to avoid thrombotic complications.

Monitoring and Long-Term Management

Patients with ITP require ongoing monitoring to assess treatment efficacy and side effects. Platelet counts, bleeding symptoms, and general health should be regularly evaluated. The management plan may need adjustments based on the patient’s response to treatment and changes in their condition or lifestyle.

Quality of Life Considerations

The impact of ITP on quality of life cannot be underestimated, as the fear of bleeding can significantly affect daily activities and mental health. Treatment decisions should, therefore, consider patient-reported outcomes and preferences.
